The author presents a single case history describing the management of a patient referred for surgical debridement of a diabetic foot ulcer which proved to be a melanoma.
Acral lentiginous melanoma is often mis-diagnosed due to a low index of suspicion and varied clinical presentation. The author describes the factors that directed him to suspect a melanoma and the subsequent biopsy. Grading of melanoma is discussed and the relevant grading systemns provided.

A well presented case history which is relevant to anyone treating patients diabetic or not.
